Background and purpose. Risk of acute ischemic stroke has been associated with carotid artery atherosclerosis as well as with periodontal disease. We studied whether oral pathology or carotid atherosclerosis was associated with the presence and quantity of bacterial DNA in their aspirated thrombi. Methods. Thrombus aspirates and control arterial blood were taken from 71 patients (70.4% male; mean age, 67.4 years) with acute ischemic stroke. Tooth pathology was registered using CT scans. Carotid stenosis was estimated with CTA and ultrasonography. The presence of bacterial DNA from aspirated thrombi was determined using quantitative PCR. We also analyzed the presence of these bacterial DNAs in carotid endarterectomies from patients with peripheral arterial disease. Results. Bacterial DNA was found in 59 (83.1%) of the thrombus aspirates (median, 8.6-fold). Oral streptococcal DNA was found in 56 (78.9%) of the thrombus aspirates (median, 5.1-fold). DNA from A. actinomycetemcomitans and P. gingivalis was not found. Most patients suffered from poor oral health and had in median 19.0 teeth left. Paradoxically, patients with better oral health had more oral streptococcal DNA in their thrombus than the group with the worst pathology (p = 0:028). There was a trend (OR 7.122; p = 0:083) in the association of ≥50% carotid artery stenosis with more severe dental pathology. Oral streptococcal DNA was detected in 2/6 of carotid endarterectomies. Conclusions. Stroke patients had poor oral health which tended to associate with their carotid artery stenosis. Although oral streptococcal DNA was found in thrombus aspirates and carotid endarterectomy samples, the amount of oral streptococcal DNA in thrombus aspirates was the lowest among those with the most severe oral pathology. These results suggest that the association between poor oral health and acute ischemic stroke is linked to carotid artery atherosclerosis....
Recent progress has been made in understanding the roles and mechanisms of endoplasmic reticulum (ER) stress in the development and pathogenesis of diabetic nephropathy (DN). Hyperglycemia induces ER stress and apoptosis in renal cells. The induction of ER stress can be cytoprotective or cytotoxic. Experimental treatment of animals with ER stress inhibitors alleviated renal damage. Considering these findings, the normalization of ER stress by pharmacological agents is a promising approach to prevent or arrest DN progression. The current article reviews the mechanisms, roles, and therapeutic aspects of these findings....
At present, the incidence of breast cancer is high worldwide and the most effective way to treat breast cancer is surgery. This study mainly explores the application of pathological three-dimensional reconstruction based on VR technology in the evaluation of the margins of breast-conserving surgery and the adjustment of radiotherapy for breast cancer. In the process of making pathological large slices, the slice markers are counted and the upper and outer cutting edges are located and placed in a large embedding box to prevent tissue deformation. ACDSEE6.0 is used to convert the aligned image BMP format into JPG format, the JPG images of each breast are cropped in batches under the same range and size, and then 3DDOCTOR1.0 is imported to edit the images. The boundary edit tool is activated to segment the target area layer by layer to create a boundary data set of the target structure. The stored file is opened and 3D rendering simple surface and volume rendering on the computer is run to display the surface data 3D display and the volume data 3D display. During VR image processing, the data are imported into the VR image workstation and the Radio Dexter software system is used for image adjustment, trimming, and sequence extraction. Then, the preliminary processed CTand MRI data are displayed in pairs. Through multiplane comparison, manual or automatic fusion tools are used to reduce the gap between image sequences. The surgical process is simulated under VR: by changing the window size and viewing angle; using virtual electric drills, cutting tools, and other virtual instruments; ad selecting the operating microscope perspective; and simulating steps such as breast opening, visual field exposure, and tumor resection. The breast tomographic image was reconstructed in multiple planes, and two-dimensional images of any plane could be observed. 10 cases showed high-density shadows and 5 cases showed isodensity shadows. This research contributes to the precise treatment of breast-conserving surgery....
Rationale. Pathological cardiac remodeling serves as a vital mechanism during the progression from myocardial infarction (MI) to chronic heart failure (CHF). Nobiletin (NOB), an active monomer extracted from the peel of citrus fruit, has been reported to have beneficial effects in cardiovascular diseases. Our study was aimed at describing the specific mechanisms through which NOB protects against pathological cardiac remodeling after MI. Materials and Methods. C57BL/6 mice were treated with coronary artery ligation to generate an MI model, followed by treatment for 3 weeks with NOB (50 mg/kg/d) or vehicle (50 mg/kg/d), with or without the peroxisome proliferator-activated receptor gamma (PPARγ) inhibitor T0070907 (1 mg/kg/d). Cardiac function (echocardiography, survival rate, Evans blue, and triphenyl tetrazolium chloride staining), fibrosis (Masson’s trichrome staining, quantitative real-time polymerase chain reaction (qRT-PCR), and western blot (WB)), hypertrophy (haematoxylineosin staining, wheat germ agglutinin staining, and qRT-PCR), and apoptosis (WB and terminal deoxynucleotidyl transferase dUTP nick-end labelling (TUNEL) staining) were evaluated. Hypoxia-induced apoptosis (TUNEL, WB) and phenylephrine- (PE-) induced pathological hypertrophy (immunofluorescence staining, qRT-PCR) models were established in primary neonatal rat ventricular myocytes (NRVMs). The effects of NOB with or without T0070907 were examined for the expression of PPARγ and PPARγ coactivator 1α (PGC1α) by WB in mice and NRVMs. The potential downstream effectors of PPARγ were further analyzed by WB in mice. Results. Following MI in mice, NOB intervention enhanced cardiac function across three predominant dimensions of pathological cardiac remodeling, which reflected in decreasing cardiac fibrosis, apoptosis, and hypertrophy decompensation. NOB intervention also alleviated apoptosis and hypertrophy in NRVMs. NOB intervention upregulated PPARγ and PGC1α in vivo and in vitro. Furthermore, the PPARγ inhibitor abolished the protective effects of NOB against pathological cardiac remodeling during the progression from MI to CHF. The potential downstream effectors of PPARγ were nuclear factor erythroid 2-related factor 2 (Nrf-2) and heme oxygenase 1 (HO-1). Conclusions. Our findings suggested that NOB alleviates pathological cardiac remodeling after MI via PPARγ and PGC1α upregulation....
Digital pathology systems (DPSs) have been globally implemented, and computer-assisted diagnosis (CAD) software has been actively developed in recent years. This study aimed to investigate perceptions of digital pathology and the demand for CAD. An online survey involving members of the Korean Society of Pathologists was conducted, and a demonstration clip of the diagnostic assistant software for a prostate needle biopsy was shown to them to provide a simple experience with CAD. One hundred sixty-four Korean pathologists (13.6% of 1210 Korean pathologists) participated. The majority (77.4%) answered affirmatively regarding the necessity of implementing a DPS, and 26.8% had plans to implement or increase the use of DPSs in the following 2–3 years at their medical institutions. Pathologists felt that multidisciplinary care or conference accessibility (56.7%), remote consultation (49.4%), and big data building (32.9%) were useful parts of DPSs. Most pathologists (81.7%) responded that CAD software would assist with the diagnostic process. In a prostate needle biopsy, pathologists used the software to improve the measurement of tumor volume and/or length and core length but not to suggest a diagnostic name or Gleason grade. Korean pathologists who participated in the survey had highly positive perceptions of digital pathology and maintained a positive attitude toward the use of CAD software....
